Each patient works closely with an alumni care coordinator and a therapist tobuild an aftercare plan that fits individual needs. With the patient’s permission, the care coordinator also connects with loved ones and professionals to strengthen support. Plans may include counseling, medical follow-up, family support, safe housing, or peer resources, helping patients stay on track after treatment.
